Become a Patient

It's easy to ask for help. Complete this questionnaire to point us in the right direction. Our process includes answering a few questions with an intake team. From there we can schedule an appointment with a psychiatric team member for an evaluation. If you meet the criteria for certain treatments, we will start the prior authorizations all while keeping in steady contact with you.

A couple of things to note: Most patients will need to utilize technology. This means checking email, using our patient portal, and responding to texts. There are work arounds for this, including coming to the office to complete the paperwork, but it must be done timely before each appointment.
